Please note that some of the following questions may not pertain to your loved one and you may not want to provide detailed information about every queston. You are in no way required to answer anything to a degree that it is cumbersome, nor provide information you feel would be inappropriate for your loved ones' video obituary tribute.

On the other hand, while we may not be able to include all the information supplied, we at OB-TV believe that the more details we recieve, and the more we know about your loved ones' unique personality and character, the better the video we will be able to produce.

After you purchase an OB-TV Package, you will need to submit the content for the tribute.

Scan and E mail the required number of photos to dpotter@ob-tv.com.
